What used to be well funded hospital based programs evolved into individual bases whose crews knew they must make money or the base will be closed. During the time when these money interests ran air medical, for crews who already risk their lives flying into harsh territory in the dark and rain it meant pressure to accept missions that otherwise they would have turned down.
